Client consent update | Collection and disclosure of personal information to other persons or organizations 

Whether it’s to ensure the privacy of your clients or to allow us to adequately serve them, protecting personal information is always a priority at iA. 

In the interest of improving our practices and complying with the current regulations, we have modified our client consent documents regarding the collection and disclosure of personal information to other persons or organizations:  

• Healthcare providers 

• Doctors, hospitals  

Here is the updated consent text, which can be found on a number of Individual Insurance, Savings and Retirement forms and sales tools (EVO Savings and Assure&go). Please note that it may differ slightly depending on the context. These changes do not affect your usual working methods. 

Privacy authorization 

I authorize iA Financial Group, its affiliates and reinsurers to collect from any healthcare professional, any public or private health or social services establishment, the Régie de l’assurance maladie du Québec, any insurance company, any financial institution, any employer or former employer, MIB LLC, any investigation agency, any group insurance administrator or any private or public organization which holds personal or medical information about me, or to communicate to them information concerning me, including my health condition, my medical history and any other information relevant to the processing of any request relating to my contract. 

The personal information that we, iA Financial Group and our affiliates, collect in connection with your request will be used and disclosed only for the purposes for which you have already consented.

Updated forms 

To ensure that you’re using the most recent versions of our forms, please always download them from the Document Centre in your Advisor Centre. 

Over the next few months, we will be modifying the EVO Insurance tool and other forms. You will be notified by email when these updates are complete.
