COVID-19 Questionnaire to be withdrawn As the impact of the global pandemic begins to ease, we’re able to update our processes to make it easier for you to do business.
The COVID-19 Questionnaire will no longer be required for any Foresters FinancialTM or Canada Protection Plan products as of June 18, 2022. This applies to new business applications and applicable in-force forms, including policy change applications and reinstatement requests.
Our e-Apps, pdf applications and forms will be updated on June 18, 2022. Please discard any old pdfs with the COVID-19 Questionnaire on that date.
