An application numbering inconsistency on the current version of the English paper Face-to-Face Life Insurance and Critical Illness Insurance Application Form 126E (2021/07/01) has created the need to re-print an updated version.
This is strictly an internal issue affecting the paper English form only and has no impact on in-flight cases.
French versions of the form, the online PDF version of the form and SmartApp are not impacted.
Effective immediately, we will begin shipping an updated version 126E (2021/12/01) as requests are received from your MGA or National Account office.
We strongly urge you to begin using the updated form but will continue to accept version 126E (2021/07/01) until further notice.
